Who Qualifies

Hospice care is for individuals facing a life-limiting illness or condition, where the focus shifts from curative treatment to providing comfort and enhancing quality of life.  Patients that qualify for hospice typically have a life expectancy of six months or less, as determined by their physician’s prognosis.

Common conditions that may make someone eligible for hospice include advanced cancer, heart failure, lung disease, dementia, and other progressive illnesses.

Our team of experienced healthcare professionals work closely with patients, their families, and the referring physicians to assess eligibility and determine the appropriate level of care.  If you have questions about hospice care or would like to discuss eligibility criteria, please don’t hesitate to reach out to us.  We are here to provide guidance and assist you in making the best decisions for your loved one’s care.
